Please bring a current medication list, your dental insurance information if applicable, and any referral or relevant dental records you have been given. If your dentist has recent imaging, ask our office how it should be transferred. Please tell us about any changes to your medical history or allergies.

If you are scheduling an oral surgery consultation with Dr. Allen Huang, please ask your general dentist to provide a written referral.
